RESPONSIBILITIES:
• Conduct thorough assessments of residents' physical, emotional, and medical needs; monitor their health closely and adapt care accordingly.
• Develop, implement, and regularly update personalized care plans in collaboration with physicians, therapists, and other healthcare professionals.
• Administer medications and treatments (oral, IV, topical, injectable) and perform skilled procedures (e.g., wound care, IV therapy, specimen collection) as per physician orders and facility policies.
• Monitor and document residents' vital signs, conduct regular health checks, and promptly identify and respond to changes or emergencies in their condition.
• Provide direct nursing care and assist residents with activities of daily living, mobility, hygiene, and other care needs.
• Work collaboratively with the interdisciplinary care team (physicians, therapists, social workers, CNAs/LPNs) to ensure cohesive, resident-focused care.
• Maintain accurate and timely documentation of assessments, care provided, and any changes in residents' conditions in their medical records.
• Engage effectively with residents, families/POAs, and the care team regarding resident status, care plans, and any concerns or changes.
• Educate and support residents and their families about care, treatments, medications, and wellness resources.
QUALIFICATIONS:
• Valid, unrestricted RN license in Connecticut.
• Graduate of an accredited nursing program (ADN or BSN); experience in long-term care or geriatric nursing is preferred.
• Current CPR/BLS (or equivalent) certification as required by facility policy.
• Exceptional clinical assessment skills, compassion, and a patient-centered approach; strong communication and interpersonal skills.
• Ability to thrive in a fast-paced long-term care or skilled nursing environment; dependable attendance and strong teamwork ethic.
• Competence in performing nursing procedures (medication administration, IV therapy, wound care), and monitoring residents' conditions, addressing emergencies as they arise.
