This full-time position offers weekdays, Monday-Friday from 8 am to 5 pm, with limited after-hours on-call responsibilities. You will have the opportunity to care for patients in the Kalamazoo County area.

Key Responsibilities of the Hospice RN/RN Case Manager:
• Serve as the primary contact for patients and families, guiding them through every step of the hospice journey.
• Administer and document skilled nursing care based on each patient’s personalized Plan of Care, whether at home, in assisted living, or other settings.
• Conduct initial and ongoing assessments to monitor patient conditions and modify care as necessary.
• Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to develop and adjust individualized care plans.
• Provide emotional, physical, and spiritual support to both the patient and their loved ones, including education, comfort, and bereavement guidance.
• Instruct caregivers on how to care for their loved ones safely and confidently, providing practical training and emotional support.
• Communicate significant updates regarding patient conditions to the care team, ensuring prompt and appropriate interventions.
• Maintain accurate and up-to-date clinical documentation while adhering to infection control and compliance standards.
• Support the organization's quality improvement initiatives and assist with scheduling to ensure consistent, compassionate, and efficient care.
• Help patients and families understand and navigate topics such as medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations.
Qualifications - What You'll Bring:
• Active RN license in your state of employment (or eligibility to obtain).
• Minimum of 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
• A heartfelt commitment to hospice care: compassionate, empathetic, and patient-centered.
• Comfortable providing care in a variety of settings, including private homes and assisted living facilities.
• Strong communication and critical thinking skills, particularly in emotionally sensitive situations.
• Valid driver's license, auto insurance, and reliable transportation are essential.
• Current CPR certification is required.
Preferred Experience (Not Required):
• Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac)
• Background in Admissions RN, wound care, float pool, or travel nursing.
• Experience working with terminally ill patients or supporting serious illnesses.
