This role is a unique opportunity provide complex primary care to individuals, focusing on preventive care and health maintenance to an ethnically and socially diverse, patient population in accordance with the values, beliefs and policies of Advance Community Health. Our interdisciplinary care teams work closely with patients, leveraging cross-sector partnerships to deliver patient-centered care tailored to each individual’s goals and priorities. Patients eligible for treatment at Beacon Point include those with complex medical issues, such as frequent hospital visits and uncontrolled medical conditions.Essential Duties and ResponsibilitiesRecord patients' medical information and vital signs.Administer medications to patients and monitor patients for reactions or side effects.Maintain accurate, detailed reports and records. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. Instruct individuals, families, or other groups on topics such as health education, disease prevention, and develop health improvement programs. Modify patient treatment plans as indicated by patients' responses and conditions. Conduct specified laboratory tests. Prepare patients for and assist with examinations or treatments. QualificationsGraduation from an accredited Bachelor's Degree in Nursing (or higher), Associate's Degree in Nursing, or Nursing Diploma program is required.Active Registered Nurse (RN) Licensure in good standing with North CarolinaBLS CertificationStrong commitment to practicing in a community health setting.Minimum of 2+ years of healthcare experience, working as an RN.Experience preferred but not required in complex care or a similar fast-paced healthcare setting.Experience with EPIC Electronic Medical Records system preferred.Excellent communication and interpersonal skills.Work EnvironmentThe work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
