Responsibilities and Requirements:
• Patient Care: Deliver efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction.
• Data Collection: Collections, validates, synthesizes, and records objective and subjective data.
• Prioritization: Prioritize data collection based on patient needs.
• Problem Identification: Analyze all data and identifies actual or potential problems or needs.
• Goal Setting: Establish realistic and measurable short and long-term goals for identified health problems and needs.
• Intervention Development: Develop individualized interventions to achieve goals.
• Timeline Determination: Determine timelines within the care plan.
• Evaluation: Conducts a systematic and ongoing evaluation of patient outcomes.
• Plan Revision: Reviews and revises the plan of care as indicated.
• Care Implementation: Implements care by performing nursing functions and providing care within scope of practice.
• Care Coordination: Coordinates and delegates care as appropriate.
• Medication Administration: Verifies that medical orders are accurately transcribed.
• Treatment Administration: Administers medications and performs treatments per physician orders.
• Communication: Communicates and documents interventions and patient response.
