Overview ORGANIZATION: Citizen Care Inc. DEPARTMENT: Life Sharing POSITION: Critical Care Nurse STATUS: Full-Time; 40 hours / week Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County. Responsibilities The Critical Care Nurse serves as a member of the nursing team reporting to the Medical Coordinator. The Critical Care Nurse will provide direct care and support to adults and children with intellectual disabilities and autism who receive support in the medically complex (MCC) life sharing program. This role is vital in ensuring health, safety, and quality of life for people and requires clinical expertise, strong communication skills, and commitment to person-centered care that aligns with the Council on Quality and Leadership (CQL) standards. Health & Wellness • Deliver direct nursing care in Life Sharing homes, including medication administration, wound care, tracheostomy and ventilator management, feeding tube management, seizure monitoring and management, respiratory treatments, and other skilled nursing tasks. • Monitor and document progress, vital signs, and changes in condition. • Ensure care delivery aligns with CQL Outcome Performance Measures, promoting quality of life and person-centered outcomes. • Collaborate with Life Sharing families, medical providers, and interdisciplinary teams to develop and implement individualized health care plans. • Conduct regular health assessments and monitor chronic and acute medical conditions. • Assist with and review medical appointments, documentation, and diagnostic testing as needed. Administrative Duties & Professional Standards • Maintain Office of Developmental Programs (ODP) Medication Administration Certification and administer medications per state guidelines. • Utilize the electronic health record (EHR) to ensure timely, accurate, and person-centered documentation in accordance with state regulations and organizational policies. This includes: • Health care summaries including monthly progress notes for children in the MCC life sharing program • Medication Administration Records (MARs) • Medical/nursing notes, social medical histories, and other clinical documentation • Maintain the content, integrity, and security of all health records (electronic and paper), ensuring documentation is trauma-informed and person-centered. • Use and integrate health records from external systems, including but not limited to EPIC. • Strong clinical judgment and critical thinking skills. • Other duties as assigned. Due to the medically complex needs of the people supported and the nature of the work being home based, successful candidates will not be able to smoke or vape during work hours. Maximum Compensation USD $34.00/Yr.
