Scheduled Hours 40 Position Summary Provides Diabetes and Nutritional educational services and community resources for patients. Maintains a case load of patients and promotes program development. Job Description Primary Duties & Responsibilities: Provides education to patients. • Provides patient education to patient/family members/significant others about plan of care. • Follows appropriate communication channels with concern and recommendations. • Provides ongoing education to patient care on new procedures, technologies and issues impacting diabetes industry. Performs assessments, planning and implementation of diabetes care measures and evaluation of patient outcomes across the continuum of care. • Collaborates with providers to facilitate patient care, develop medical plans of care and follows appropriate communication channels with patient concerns and recommendations. ⋠â‹Provides services to patients to ensure continuous quality improvement. • Participates in quality improvement process to facilitate changes within the clinic. • Remains aware of patient satisfaction results and problem solves with patients. • Identifies patient dissatisfaction and intervenes appropriately. • Preserves patient privacy and dignity. Initiates development of new ideas and methods to provide cost-effective care. • Coordinates patient care services with other staff and departments to decrease unnecessary cost. • Assists providers in reducing costs and increasing effeciency while maintaining high quality of care. • Maintains diabetes supplies and medication appropriately. ⋠Provide education and training to staff to ensure adequate knowledge and skills to perform essential responsibilities of their positions. Perform other duties as assigned. Basic Life Support - American Heart Association, Basic Life Support - American Red Cross, Board Certified-Advanced Diabetes Management - Association of Diabetes Care and Education Specialist, Certified Diabetes Care and Education Specialist (CDCES) - Certification Board for Diabetes Care and Education, Registered Nurse - Illinois Department of Financial and Professional Regulation, Registered Nurse - Missouri Division of Professional Registration Work Experience: Relevant Experience (3 Years) Skills: Not Applicable Driver's License: A driver's license is not required for this position. More About This Job Required Qualifications: • CDCES Certified Diabetes Care and Education Specialist with three yearsâ™ experience. • Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements). Preferred Qualifications: • Five yearsâ™ experience. • Ability to flex one evening a month for evening class.  • Pump certified- Minimed/Tandem/Omnipod. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
