As the Director of Case Management for our Acute Care Services. This role leads the strategy, operations, and performance of our case management program—driving evidence-based care, reducing clinical variation, and improving outcomes through strong multidisciplinary collaboration.

Responsibilities

Direct case management services, workflow, and resources.
• Formulate and implement operational strategies and initiatives in the case management program.
• Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.
• Provide specialized expertise, ensure resource alignment with system-wide initiatives, and updates appropriate parties on case management strategies, initiatives, outcomes and issues.
• Partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care. Collaborate with a larger multidisciplinary team to improve quality outcomes using data and documented best practice.
• Responsible for resolving escalated issues arising from operations in the case management department.
Requirements

Licensure / Certification / Registration:
• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.

Education:
• Bachelor's degree in nursing or diploma from an accredited school/college of nursing required.
• Master's degree or combination of Bachelor's degree and experience preferred. OR 5 years of experience required.
• 10 years of experience preferred.
• 2 years of leadership or management experience required.
• 5 years of leadership or management experience preferred.
Additional Preferences
• Experience: Minimum of five (5) years of Case Management experience within an Acute Care hospital environment.
• Leadership: A minimum of two (2) years of proven leadership or management experience, preferably overseeing Case Management or Utilization Review teams in an Acute Care setting.
• Certification: Current Case Manager certification from a recognized body, such as the American Case Management Association (ACMA) or the Commission for Case Manager Certification (CCMC).
