In This Role You Will: The Director of Nursing- Home Health • Oversee and direct all clinical services to ensure high-quality, compliant patient care. • Review and approve plans of care, patient eligibility, and service levels. • Supervise clinical staff, including performance management, education, and complaint resolution. • Coordinate care assignments and interdisciplinary services based on patient and physician needs. • Collaborate with the Executive Director on clinical operations, staffing, and quality initiatives. • Participate in audits, surveys, quality improvement, and regulatory compliance activities. • Support recruitment, orientation, and ongoing staff development. • Maintain strong relationships with physicians, referral sources, and community partners. • Provide on-call coverage, including evenings and weekends, as assigned. Shift Details: This is a full-time, weekday leadership position with standard business hours. The Director of Nursing participates in a rotating on-call schedule and is expected to remain available to support clinical needs as required, including evenings, weekends, and holidays. Travel within the service area may be required. Who You Are: • Current Registered Nurse (RN) license in the state of Indiana. • Minimum of 1–2 years of leadership nursing experience (home health preferred). • Knowledge of OASIS documentation and Medicare home health regulations preferred. • Strong clinical assessment, critical thinking, and problem-solving skills. • Excellent organizational and communication skills.
