• Shifts will be from 9:00 AM - 5:30 PM
• 3 x 8hr shifts per week requirement = 24 hrs/ week
• Position is ineligible for benefits or incentives.
Discharge planning is critical for preparing the patient to leave the hospital. The Discharge Nurse is proficient and knowledgeable of Orlando Health’s Discharge process. The Discharge Nurse helps to make sure that the patient and their family understand exactly what to do after discharge to prevent injury and encourage healing. They ensure the patient has the correct contact information for patient follow-ups and makes phone calls to arrange for follow-up services, equipment, and supplies, as well as reinforce patient instructions and preparations for discharge. Proper discharge planning will also help avoid the need for patient re-hospitalization in the near future. Therefore, this nurse must work closely with the patient’s doctors and nurses to ensure that there is a plan for discharge and that everyone involved knows what they are.

Responsibilities
Essential Functions
• Collaborate with the patient’s nurse and check their chart to see what you need to prepare. You’ll need to consider the following:
• Where are they going? Will the patient be returning directly home or to some other care facility? Additionally, you should make sure they have transportation to get there.
• Do they need equipment? If the patient is returning home and needs further care, it’s possible that they’ll need medical equipment at home. This could be glucose monitoring items, a wheelchair, or medications. You’ll need to make sure they have this already at their home or that they have a way to get it.
• Complete Discharge Paperwork
• Medication Teaching on medications the patient will be going home on
• Perform Discharge Teaching/Education with patient and family
• Transport qualified patients to Discharge Lounge
Qualifications
Education/Training
• Graduate of an approved school of nursing.
Licensure/Certification
• Maintains current State of Florida RN license or valid eNLC multistate RN license.
• Maintains current BLS/Healthcare Provider certification.
Experience
• 2 years as a Registered Nurse in a hospital setting with Epic discharge experience
