Experienced Registered Nurse (Transition Care) - Full Time in Knoxville, TN

**Overview:** Fort Sanders Regional Medical Center, a 444-bed hospital recognized for its exceptional care and advanced medical technology, is seeking an experienced registered nurse to join its transition care team. As a member of our team, you will have the opportunity to work with a diverse patient population and provide high-quality care in a supportive and collaborative environment. **Responsibilities:** • Correlate patient history and support systems within hospital care/nursing home care • Assess the needs of patients and families • Develop and initiate plan of care, involving patients and families • Recommend interventions and evaluate the plan of care • Provide clinical expertise and knowledge to resolve complex patient/management/physician problems without supervision • Lead process improvement initiatives in your own unit or participate in facility teams • Precept new employees/students and review clinical performance with unit leaders or clinical instructors • Provide four contact hours of educational activities, excluding basic patient education • Contribute to development and evaluation of unit orientation programs • Supervise LPNs, Nursing Assistants, HUCs, Techs, and sitters • Maintain a safe environment for all patients, reporting any concerns to managerial staff • Follow policies, procedures, and safety standards, and participate in quality improvement initiatives • *Qualifications:** • Minimum of three (3) years of professional experience • Valid Tennessee RN license • CPR certification • *Benefits:** • Competitive salary and benefits package • Tuition reimbursement and student loan assistance • Certification bonuses and leadership development programs • Opportunity to work with a top-ranked hospital and health system • Collaborative and supportive work environment • Professional development opportunities to advance your career
