CentraCare – Clearwater is seeking a Family Medicine Advanced Practice Provider (NP or PA) to join our collaborative primary care team. You will provide comprehensive, team-based care that supports the Quadruple Aim — improving patient outcomes, enhancing patient and provider experiences, and ensuring cost-effective care.Your Schedule.8 – 1.0 FTE (flexible based on your needs)Choose a 4- or 5-day workweekClinic hours:Monday: 7:30 a.m. – 5:00 p.m.Tuesday–Friday: 8:00 a.m. – 5:00 p.m. (with flexibility as needed)No evenings, weekends, holidays, or callYour RoleAs a trusted primary care provider, you will:Diagnose and manage acute and chronic conditionsDeliver preventative and wellness care, including routine checkups, screenings, and immunizationsProvide personalized health counseling and educationCoordinate care across specialties for comprehensive patient supportWhat We’re Looking ForGraduate of an accredited Nurse Practitioner or Physician Assistant program1+ year of clinical experience as an NP or PA preferredCurrent or eligible Minnesota licensureActive AHA BLS certification and DEA registration (or ability to obtain prior to start)Experience in any of the following settings is a plus:Family MedicineInternal MedicineUrgent CareEmergency MedicinePediatrics Pay and BenefitsStarting pay begins at $109,241.60 per year; exact wage determined by years of related experience Salary range: $109,241.60 - $124,904 per year Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rateFull-time benefits: medical, dental, PTO, retirement, employee discounts and more!
