In this Field-Based RN Health Coordinator role, you'll coordinate the long-term care needs of patients within the community. With the backing of a dynamic and skilled team, you'll be equipped to take on this rewarding challenge at UnitedHealth Group. Your role is pivotal to our Integrated Care Model, where you'll relay crucial information about member needs, advocate for quality care, and ensure that each patient receives tailored services that fulfill their requirements. This position allows you the flexibility to work remotely while traveling up to 75% of the time within the West Hawaii community, including areas such as Kailua Kona, South Kona, and Ocean View. Primary Responsibilities: • Assess, plan, and implement individualized care strategies focused on providing the least restrictive level of care. • Identify and initiate referrals for essential social service programs, including financial, psychosocial, and community support services. • Manage care plans across the continuum as a single point of contact. • Communicate vital health-related information among all stakeholders to ensure high-quality, coordinated care. • Advocate for patients and families to ensure their needs and choices are represented and supported. Required Qualifications: • Current, unrestricted RN license in the state of Hawaii. • At least 2 years of clinical experience. • Intermediate proficiency in MS Word, Excel, and Outlook. • Access to a reliable Internet connection via a landline. • Willingness to travel within your assigned region to meet Medicaid members in diverse settings, including their homes, community centers, and hospitals. Preferred Qualifications: • Bachelor's degree or higher. • Experience in long-term care, home health, hospice, public health, or assisted living. • Expertise in case management or care coordination. • Experience arranging community resources. • Prior field-based work experience. • Familiarity with electronic charting systems. • Background in managing populations with complex medical or behavioral needs. All remote employees must adhere to UnitedHealth Group's Telecommuter Policy. Compensation for this role will range from $28.94 to $51.63 per hour, depending on various factors.
