Position Responsibilities: The Care Coordinator 2 utilizes various strategies to effectively manage the physical, environmental, and psycho-social health aspects of our members. • Build trusting, long-term relationships with each member to enhance the quality of care and coordination. • Coordinate required medical and non-medical services, addressing members' functional, social, and environmental needs. • Collaborate with Service Coordinators and Transition Coordinators to meet the identified needs of members. • Interface seamlessly with all Medicare payers and providers for members eligible for both Medicaid and Medicare services. • Act as the primary contact for the Interdisciplinary Care Team, ensuring all member needs are communicated effectively among team members and external resources. Your Skills Matter! Required Qualifications: • Active Registered Nurse (RN) license in Indiana with no restrictions. • A minimum of two years of clinical experience in case management or comparable healthcare services. • Intermediate to advanced computer skills, particularly proficiency in Microsoft Word, Excel, and Outlook. • Exceptional communication skills with the ability to build rapport with diverse stakeholders. • Strong critical thinking, organizational, and problem-solving skills. • Ability to prioritize effectively in a fast-paced environment. • Experience with electronic medical records and various applications/software. • Must reside in Indiana. Preferred Qualifications: • Bilingual in English/Spanish or English/Burmese. • Prior experience in nursing home diversion, long-term care, or case management. • Experience managing Home and Community Based Services waivers. • Previous work with Medicare & Medicaid recipients. • Familiarity with senior populations and health promotion initiatives. • Knowledge of community health and social service agencies. Workstyle: Hybrid model that includes both remote work and onsite member visits. Location: Must reside in Indiana. Hours: Standard 40-hour work week, Monday through Friday, 8:00 AM to 5:00 PM; overtime may be required as necessary. Travel: Willingness to travel approximately 70% for member meetings is essential. On Call: Telephonic on-call responsibilities for occasional nights/weekends may be required. Technical Requirements: • Self-provided internet service must have a minimum of 25 Mbps download and 10 Mbps upload speed; a wired connection is preferred. • Satellite, cellular, and microwave connections require prior approval. • Humana will provide necessary phone equipment to meet job requirements. • Work from a distraction-free area to ensure protection of member PHI/HIPAA information. Driver's License and Insurance: As part of Humana's Driver Safety program, you must have: • A valid state driver's license. • Proof of personal vehicle liability insurance with minimum limits of $100,000/$300,000/$100,000. • Access to a reliable vehicle. Tuberculosis (TB) Screening: Participation in Humana's TB screening program is required for patient-facing roles. Interview Process: Our hiring process includes a screening technology that enhances decision-making by allowing you to share your experience via text, video, or voice formats at your convenience. This position may require occasional travel to Humana offices for training or meetings. Scheduled Weekly Hours: 40 Compensation: The estimated salary range for this full-time position is $71,100 - $97,800 per year. Compensation may vary based on geographic location and individual qualifications. This role is also eligible for a performance-based bonus incentive plan.
