We support both patients and their families with dignity, comfort, and compassion. Flexible PRN Registered Nurse – Day and Hour Flexibility! What You Will Do as a Hospice RN / RN Case Manager: • Act as the primary point of contact for patients and their families, guiding them through every step of the hospice journey. • Deliver and document skilled, hands-on nursing care tailored to each patient's individualized Plan of Care, whether it's in their home, assisted living, or any place they call home. • Conduct initial and ongoing assessments to monitor and adapt patient care as necessary. • Collaborate with an interdisciplinary team, including physicians, LPNs, CNAs, social workers, chaplains, and others to develop and enhance personalized care plans. • Provide comprehensive emotional, physical, and spiritual support not only to the patient but also to their loved ones, offering education, comfort, and bereavement support. • Guide caregivers in ensuring safe and confident care for their loved ones through practical training and emotional reassurance. • Communicate critically important updates on patient conditions to the care team, ensuring timely and appropriate interventions. • Maintain accurate and up-to-date clinical documentation while adhering to infection control and compliance standards. • Support the organization's quality improvement initiatives to guarantee that care is compassionate, consistent, and efficient. • Educate patients and families on navigating complex topics such as medication administration, hospice philosophy and services, symptom management, and end-of-life expectations. About You – Your Qualifications: • Active RN license in the state of employment (or eligible to obtain). • At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields. • A genuine passion for hospice care: compassionate, empathetic, and patient-centered. • Comfortable providing care in a variety of settings, including private homes and facilities. • Strong communication and critical thinking skills, particularly in emotionally sensitive situations. • Valid driver’s license, auto insurance, and reliable transportation required. • CPR certification is mandatory. Preferred Experience (Not Required): • Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac). • Admissions RN, wound care, float pool, or travel nursing experience. • Experience caring for terminally ill patients or providing serious illness support.
