Job Responsibilities
Direct Client Care
• Conduct initial assessments, re‑assessments, and safety evaluations for skilled and unskilled home care clients.
• Deliver skilled nursing care within the Ohio Nurse Practice Act, including infusion therapies (IV/IG), wound care, injections, and other clinical interventions.
• Develop, update, and maintain person‑centered Plans of Care; obtain timely physician signatures.
• Observe, document, and report changes in client condition; follow infection control protocols and universal precautions.
• Serve as a strong patient advocate and field‑based clinical resource.
Response Readiness & Team Support
• Maintain a flexible 40‑hour workweek within Monday-Friday, 7:00 AM-7:00 PM (overtime and some weekend hours available).
• Be prepared to leave for a shift within 30 minutes during on‑call or urgent‑need periods (BrightStar will schedule shifts in advance whenever possible).
• Maintain readiness at all times by ensuring:
• Uniform and ID badge are worn.
• Childcare is pre‑arranged.
• Cell phone is charged with ringer on.
• Reliable transportation is available.
• Required equipment and supplies are accessible.
• Support office operations when not assigned to field shifts, contributing to a collaborative, high‑performing team environment.
Administrative & Clinical Support
• Partner with the Clinical Director on administrative and compliance responsibilities, including:
• Weekly chart reviews to ensure adherence to state regulations, Joint Commission (JCAHO) standards, and BrightStar brand requirements.
• Verification of signed physician orders, authorizations, and supervision of high‑risk medications.
• Incident reporting, follow‑up, and resolution.
• Timely and thorough client discharges with complete summaries.
• Daily documentation review to support accurate billing and reimbursement.
• Coordinate care with families, providers, referral sources, and internal staff.
• Assist with referral intake and follow‑up communications.
• Conduct caregiver and client introductions as needed.
• Complete supervisory visits for skilled and non‑skilled staff to ensure compliance and quality.
• Lead and support field staff education, including weekly orientation, competency demonstrations, skills training, and ongoing in‑services.
• Provide guidance and support for urgent clinical questions or issues.
Job Requirements
Licensure & Education
• Current, unrestricted Registered Nurse (RN) license in the State of Ohio
• Degree in Nursing from a National League for Nursing-accredited program
• Minimum of three (3) years of clinical experience, including at least one (1) year of documented case management
• Preferred: One (1) year of experience in a home health agency or healthcare management
• Preferred: Experience working with pediatric clients
Clinical Competencies
• Strong nursing assessment skills with exceptional attention to detail
• Ability to supervise, educate, and direct skilled and unskilled staff
• Thorough understanding of state and federal regulatory compliance
• Familiarity with Joint Commission (JCAHO) standards and survey readiness
• Adherence to HIPAA and commitment to maintaining client confidentiality
• Ability to manage complaints, incidents, and complex clinical situations using sound judgment
• Experience in client teaching, staff coaching, and public speaking
• Proven ability to implement quality improvements within clinical operations
Interpersonal & Professional Skills
• Impeccable communication skills; fluent in reading, writing, and speaking English
• Excellent customer service and tactful communication with clients, families, staff, and community partners
• Strong interpersonal skills including empathy, active listening, leadership, and motivation
• Ability to multi‑task, troubleshoot, and remain composed in high‑pressure or rapidly changing situations
• Comfortable working independently and collaboratively within a team
• Technologically proficient (Microsoft Office, EMR platforms, mobile apps)
Availability & Readiness Requirements
• Flexible schedule with availability for on‑call coverage as needed
• Willingness to travel daily throughout the assigned territory, including occasional weekends, evenings, and holidays
• Valid driver's license, reliable transportation, and current auto insurance
• Ability to maintain readiness for rapid response (e.g., transportation, childcare, equipment, communication)
Health & Safety Requirements
• Current CPR certification
• Negative TB test within the past 12 months (or chest X‑ray as required)
• Up‑to‑date vaccinations: Influenza, COVID‑19, Tetanus, Diphtheria, Pertussis, Rubella, Varicella, MMR, Hepatitis B
• Ability to pass drug screening, background check, and criminal record clearance
• Physical ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull
• Ability to lift up to 50 pounds; varied assistance required depending on client mobility
