What You'll Do as a Hospice RN / RN Case Manager: • Act as the primary point of contact for patients and families, guiding them through their unique hospice journey with support and care. • Deliver and document skilled nursing care tailored to each patient's individualized Plan of Care, providing services in their own homes, assisted living facilities, or other locations. • Conduct initial and ongoing assessments to monitor patient conditions and modify care plans as necessary. • Collaborate with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, and chaplains—to create and refine customized care plans. • Offer emotional, physical, and spiritual support not just to patients but to their families, providing education, comfort, and bereavement guidance. • Train caregivers to confidently and safely assist their loved ones, offering both practical instruction and emotional assurance. • Communicate significant updates regarding patient conditions to the care team to ensure timely and effective interventions. • Maintain accurate clinical documentation and adhere to infection control and compliance standards. • Contribute to the organization's quality improvement and scheduling efforts, ensuring that care is compassionate and efficient. • Assist patients and families in understanding and navigating topics such as medication administration, hospice philosophy, symptom and pain management, and end-of-life expectations. Qualifications - What You'll Bring: • Active RN license in the state of employment (or eligibility to obtain it). • A minimum of 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar fields. • A genuine hospice heart: compassionate, empathetic, and centered on patient needs. • Comfort in providing care in varied settings, including private homes and healthcare facilities. • Strong communication and critical thinking skills, especially in emotionally charged situations. • Valid driver's license, auto insurance, and reliable transportation are essential. • CPR certification is required. Preferred Experience (Not Required): • Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac). • Roles such as Admissions RN, wound care, float pool, or travel nursing. • Experience with terminally ill patients or support for serious illnesses.
