
Full-Time Hospice Registered Nurse (RN) in Phippsburg, ME

Your Role as a Hospice RN/RN Case Manager:
• Act as the primary point of contact for patients and families, guiding them through each step of their hospice journey.
• Deliver skilled, hands-on nursing care tailored to each patient's individualized Plan of Care in their preferred environment—be it home, assisted living, or elsewhere.
• Conduct initial and ongoing assessments to monitor patient health, adjusting care plans as necessary.
• Collaborate with an interdisciplinary team including physicians, LPNs, CNAs, social workers, chaplains, and others to create and enhance personalized care plans.
• Provide emotional, physical, and spiritual support to patients and their families, offering education, comfort, and bereavement guidance.
• Teach caregivers practical skills to confidently and safely care for their loved ones.
• Communicate crucial updates regarding patient condition to the care team, ensuring timely and appropriate interventions.
• Maintain precise and current clinical documentation while adhering to infection control and compliance standards.
• Support organizational quality improvement and scheduling efforts to ensure care remains consistent, compassionate, and efficient.
• Assist patients and families in understanding and navigating important topics such as medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes.
About You:
• Active RN license in the state of employment (or eligibility to obtain one).
• 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar nursing fields.
• A genuine passion for hospice care: compassionate, empathetic, and patient-centered.
• Comfortable providing care in diverse environments, including private homes and facilities.
• Strong communication and critical thinking skills, especially in emotionally sensitive situations.
• Valid driver's license, auto insurance, and reliable transportation.
• CPR certification is required.
Preferred Experience (Not Required):
• RN experience in hospital settings (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
• Admissions RN, wound care, float pool, or travel nursing experience.
• Experience caring for terminally ill patients or providing serious illness support.
