Your Role as a Hospice RN/RN Case Manager:
• Act as the primary contact for patients and their families, guiding them through each step of their hospice journey.
• Provide and document skilled nursing care tailored to each patient's individualized Plan of Care in their home or preferred residence.
• Conduct initial and ongoing assessments to evaluate patient conditions and make necessary adjustments to care plans.
• Collaborate with a multidisciplinary team, including physicians, LPNs, CNAs, social workers, and chaplains, to develop and refine personalized care plans.
• Offer emotional, physical, and spiritual support to patients and their loved ones, providing education, comfort, and bereavement guidance.
• Educate caregivers on how to care for their loved one safely and confidently, offering practical training and emotional support.
• Communicate significant updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
• Maintain thorough and up-to-date clinical documentation while adhering to infection control and compliance standards.
• Participate in the organization’s quality improvement initiatives to guarantee that care is consistent, compassionate, and efficient.
• Assist patients and families in understanding and navigating topics such as medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes.
About You:
• Active RN license in the state of employment (or eligibility to obtain one).
• At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
• A true passion for hospice care: compassionate, empathetic, and patient-centered.
• Comfortable providing care in diverse environments, including private homes and care facilities.
• Strong communication and critical thinking skills in sensitive situations.
• Valid driver's license, auto insurance, and reliable transportation.
• CPR certification required.
Preferred Experience (Not Required):
• Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
• Experienced in Admissions RN, wound care, float pool, or travel nursing.
• Experience caring for terminally ill patients or in serious illness support.
