In the role of Registered Nurse Case Manager, you will take charge of managing the nursing care plan for hospice patients. You will continuously assess your performance to enhance productivity and the quality of care provided. Collaborate with a team of professionals to create and implement a comprehensive care plan that addresses the physical, emotional, and spiritual needs of patients and their families during this critical time.
• This is a Full-Time position conducting Home Visits for Hospice Care.
• RN license in Virginia is required.
• Our Business Hours are Monday through Friday, 8 AM to 4:30 PM, with an on-call rotation for evenings and weekends as required.
• The agency serves a 50-mile radius; candidates must be willing to reach patients within this area.
Key Responsibilities:
• Conduct initial evaluations to determine hospice service eligibility within 48 hours of referral.
• Assess and identify the physical, psychosocial, and environmental needs of patients/families, with reassessments at least every 15 days.
• Initiate and coordinate the patient's care plan.
• Document issues, goals, interventions, and responses to care appropriately.
• Work collaboratively with patients, families, physicians, and the interdisciplinary team (IDG) to ensure comprehensive care.
• Guide and supervise patients and families in self-care as appropriate.
• Maintain accurate clinical notes reflecting the patient's condition and progress.

Required Qualifications:
• Current and unrestricted RN licensure in the state of Virginia.
• Minimum of 1 year of clinical experience.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and reliable transportation or access to public transport.
• Ability to provide care in diverse home environments, respecting patients' various backgrounds.
State-Specific Requirements:
• Experience with terminally ill patients is required in Virginia.
Preferred Qualifications:
• Able to work independently.
• Strong communication, writing, and organizational skills.
Your pay is determined by several factors, including local labor markets, experience, and education, with a salary range of $58,800 to $105,000 annually for full-time employees.
