
Full-Time Post-Acute Care RN (Long-Term Care Center) in Albertson, NY
Job Description
FlexStaff is seeking Full-Time Post-Acute Care RNs for a direct-hire opportunity with our agency client, a dedicated long-term care rehabilitation facility. We have openings available in Brooklyn, NY, and Staten Island, NY.
In this role, you will provide exceptional post-acute care while working closely with residents and their families to deliver personalized support as they work toward recovery.

Key Responsibilities
• Provide comprehensive nursing care to residents in accordance with established standards and practices
• Develop, implement, and evaluate individualized care plans for each resident
• Monitor and assess residents' conditions, documenting changes and communicating effectively with the healthcare team
• Administer medications and treatments as prescribed and monitor resident responses
• Collaborate with interdisciplinary teams to coordinate care and enhance the residents' rehabilitation experience
• Educate residents and their families about care plans, therapies, and health maintenance
• Ensure compliance with healthcare regulations and facility policies
• Participate in quality improvement initiatives and ongoing professional development
Qualifications
• Valid RN license in New York State
• Bachelor of Science in Nursing (BSN) preferred
• A minimum of 2years’ experience in post-acute, long-term care, or rehabilitation nursing
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a team-oriented environment
• Compassionate, patient-centered approach to care
