
Full-Time Registered Nurse in Atlantic Beach, FL
Job Description
As a Registered Nurse in Home Health, you will provide compassionate and professional nursing care directly to patients in their homes, working closely with physicians and adhering to regulatory and agency standards. You will coordinate a comprehensive care plan with other healthcare professionals, enhancing the continuity of care for your patients.
Primary Responsibilities:
• Clinical Competence:
• Initiate, develop, implement, and revise care plans in collaboration with physicians and healthcare teams.
• Supervise and instruct home health aides and licensed practical/vocational nurses, delegating tasks in line with regulations.
• Conduct required supervisory visits to ensure quality care.
• Documentation and Care Delivery:
• Deliver high-quality clinical services following infection control and care standards, coordinating with the healthcare team.
• Complete comprehensive assessments (OASIS) including accurate medication reconciliation.
• Document patient visits as per policy and payer requirements, ensuring timely synchronization with LHC policies.
• Quality Assurance:
• Conduct evaluations to ensure eligibility for home health services, working with physicians to establish care plans based on guidelines.
• Communicate effectively with agency staff regarding patient care, visit schedules, and coordinate with clinicians.
• Maintain open communication with physicians, patients, and families to ensure outstanding care delivery.
• Teamwork:
• Follow directions from the Clinical and Executive Directors professionally, completing assigned tasks promptly.
• Participate in the orientation of new team members and serve as a mentor to peers and students.
• Engage in performance improvement initiatives to enhance patient outcomes and reduce unnecessary hospitalizations.
• Be available for on-call and weekend shifts as needed to support patient care.
• Adhere to the agency's utilization management strategy.

Required Qualifications:
• Active RN licensure without restrictions in your practicing state.
• Minimum of 1 year experience in Home Health.
• Current Driver's License, vehicle insurance, and reliable transportation.
• Current CPR Certification.
• Capability to work in diverse home environments without bias.
Preferred Qualifications:
• Ability to operate independently.
• Strong communication, writing, and organizational skills.
Your compensation will be competitive, ranging from $28.27 to $50.48 per hour based on multiple factors including experience and local market conditions.
