Responsibilities: • Conduct comprehensive assessments of residents' health conditions and adjust care as necessary. • Collaborate with healthcare professionals to develop and update individualized care plans. • Administer medications and treatments as directed by physicians. • Monitor residents' vital signs and address any changes or emergencies promptly. • Provide supportive care in daily living activities and assist with mobility and hygiene. • Maintain clear documentation of nursing assessments, care provided, and resident interactions. • Educate residents and families about care and wellness. Qualifications: • Unrestricted RN license in Connecticut. • Graduate of an accredited nursing program (ADN or BSN); experience in long-term care is preferred. • Current CPR/BLS certification as required. • Strong clinical and interpersonal skills, emphasizing a patient-centered approach. • Ability to thrive in a fast-paced nursing environment.
