
Full-Time RN Clinical Manager - Hospital at Home Program in Florida Ridge, FL
Job Description
Location: Stuart to Vero Beach, FL (Hybrid)
Schedule: Monday-Friday, 8:00 AM - 5:00 PM
Salary: $72,000 annually + Benefits
About the Position
We are seeking an experienced and motivated Registered Nurse (RN) Clinical Manager to lead our Hospital at Home Program.
This full-time hybrid position combines both field-based patient visits throughout the Stuart-Vero Beach area and administrative leadership responsibilities. The ideal candidate will have prior experience with Hospital at Home models and meet onboarding requirements specific to this program.
As the RN Clinical Manager, you will play a key role in overseeing patient care coordination, supporting field clinicians and caregivers, and ensuring the highest standards of clinical excellence. Key Responsibilities
• Manage case coordination, including patient intake, authorizations, and care planning for skilled and non-skilled services.
• Oversee daily clinical and administrative operations to ensure smooth program function.
• Conduct start-of-care and supervisory visits, as well as orientations and training for home health aides.
• Provide clinical oversight in accordance with nursing best practices and agency policies.
• Mentor and support field staff to ensure consistency and quality of care delivery.
• Maintain compliance with all state, federal, and accreditation standards.
• Collaborate with referral partners, physicians, and interdisciplinary team members to promote excellent patient outcomes.
Qualifications
• Active RN license in the State of Florida (required)
• At least one year of home care experience (preferred)
• Experience with Hospital at Home programs (highly preferred)
• Valid driver's license and reliable transportation
• Strong communication, leadership, and time management skills
• Proficient in EMR systems; experience with AlayaCare a plus
