A company is looking for a Care Navigator Acute. Key Responsibilities Participates in patient care rounds and coordinates communication among healthcare team members to ensure effective discharge planning Assesses patients' needs and develops discharge plans, utilizing internal and external resources for safe transitions of care Monitors patient progress, documents care, and advocates for necessary funding and treatment alternatives Required Qualifications Completion of an accredited nursing educational program; BSN preferred Current registered nursing license; eligibility for compact license if managing patients outside the state Minimum of three years of recent experience in acute care, home health, case management, or discharge planning Experience as a Care Manager preferred Familiarity with IT solutions such as electronic health records and care management systems is a plus
