Position: GIP Nurse Liaison Category: Business Development Type: Company Employee Status: Full-time Schedule: Monday - Friday, 8:30am-5:00pm, some weekends Work Location: Hospital Partners in Montgomery County FLSA Classification: Exempt Benefits Eligible- to review benefits, please visit our website.: https://montgomeryhospice.org/about-us/careers/ Montgomery Hospice is seeking a compassionate and experienced Nurse Practitioner (NP) to join our interdisciplinary hospice and palliative care team. This role involves direct patient care in home, hospital, and inpatient hospice settings, including Face-to-Face visits, symptom management, and coordination with care teams to ensure the highest standard of end-of-life care. Position Summary The Nurse Liaison works collaboratively with clinical teams to ensure smooth and timely transitions for referred patients, serving as a key point of contact between the referral source, the patient, and the hospice team. They are responsible for accurately tracking referral activity and maintaining up-to-date records in the Electronic Medical Record (EMR) system. Additionally, the GIP Nurse Liaison is responsible for evaluating patients referred to hospice, determining clinical eligibility, completing the admission process for non-transferable GIP eligible patients, and initiating the plan of care and case managing the GIP patients in collaboration with the interdisciplinary team (IDT). The GIP Nurse Liaison is responsible for managing qualified hospice referrals to meet or exceed quarterly marketing and admissions goals. This field-based role involves frequent engagement with physicians, hospital staff, patients, and families at assigned hospital. Through scheduled visits, in-services, and presentations, the GIP Nurse Liaison educates healthcare professionals and community partners about hospice services to drive referrals and support organizational growth. Qualifications: • Graduation from an accredited nursing program; BSN preferred. • Minimum of three years’ experience working in healthcare required. Some experience working in a hospice environment preferred. • Active and current nursing license from Maryland Board of Nursing or MultiState licensure. RN’s assigned to D.C. hospitals are required to have D.C. license. • Must possess a valid driver’s license and have a clean driving record. • Must have knowledge of state, federal and regulatory standards, including accrediting body standards, as applied to hospice organizations. • Must have effective interpersonal skills, excellent written and oral communication skills. • Access to and ability to drive a car for local travel to appointments. • Computer proficiency required. Duties and Responsilbilites: • Primary Job Duties and Responsibilities • Understands the diagnostic criteria for GIP eligibility and effectively applies knowledge when working with prospective families, patients, referring providers and hospital staff. • Receives and coordinates referrals for GIP patients at assigned hospital. • Presents Casey House as the primary GIP option and assesses patient condition to determine appropriateness for transfer. • Processes home hospice referrals for patients in the assigned hospital, including evaluating eligibility, coordinating with the care team, and ensuring a smooth transition to hospice services. • Actively participates in rounds to assist in identifying patients that would benefit from hospice referral. • Effectively holds goals of care and end of life care conversations with patients, families, or other caregivers using indepth knowledge of disease state and prognosis of illness to support patient/family decision making regarding care options and support. • Provides admission assessment and immediately engages with patient to develop rapport and a sense of trust. • Works collaboratively with hospital team when discharging and readmitting a patient in the acute care setting to GIP • Collaborates with patient/family when developing plan of care, adjusting communication style as necessary. Establishes priority for patient care based on immediate patient condition and mutually set patient needs/expectations. • Performs consent visits as scheduled to provide information regarding Montgomery Hospice, Inc. services and obtains signed consents. • Communicates relevant information to the provider, including any change in referral diagnosis. • Case Manages patients who are GIP level of care which includes performing a daily Skilled Nursing Visit and Assessment. • Conducts death visit when appropriate and assists in the assessment of family needs for bereavement services following the patient's death. Conveys information to bereavement staff no later than three days after death of patient. • Meets with providers and hospital staff to assure services are meeting needs of referred patients and families. • Addresses concerns and complaints quickly. • Demonstrates knowledge of regulatory requirements and ensures compliance with all requirements.
