Health Coach RN – Fully Remote (Contract Role)

Overview

We are seeking skilled Registered Nurses to join our team as Health Coach RNs. This fully remote position focuses on telephonic outreach, care coordination, education, and support for members following inpatient stays or those managing chronic conditions.

This is a member-facing role ideal for nurses who can blend strong clinical judgment with excellent communication and technical proficiency.

Key Responsibilities
• Conduct outbound telephonic outreach to members post-discharge and those with chronic medical needs.
• Support the inbound queue and handle member questions and concerns.
• Provide education on care plans, chronic conditions (diabetes, oncology, etc.), and benefits.
• Utilize claims-based information to guide interventions and next steps.
• Document all interactions accurately and meet performance expectations related to enrollment, outreach, audits, and productivity.
• Collaborate closely with team members in a virtual environment.

Required Qualifications
• Active RN license (PA unrestricted or Compact)
• Certified Case Manager (CCM) – required
• 3+ years of nursing experience, including:

Case Management

Home Health

Medical-Surgical
• Experience with oncology or diabetes populations preferred
• Strong computer skills, including proficiency with Microsoft Office
• Comfort working with adult populations
• Background may be from hospital or insurance settings
