As an RN Case Manager, you will serve as both a skilled clinician and a care coordinator — visiting patients in their homes to provide high-quality nursing care while managing their overall plan of care.You’ll work closely with patients, families, physicians, and our interdisciplinary healthcare team to ensure the best possible outcomes. Requirements • Current Illinois RN license in good standing. • Associate’s degree in nursing required; BSN preferred. • At least 1 year of home health experience (preferred). • Current CPR certification. • Strong communication, leadership, and critical thinking skills. • Valid driver’s license and reliable transportation. • Proficiency with electronic documentation (Kinnser experience a plus).
