Job specification Candidate requirements Job specification • Educate and train patients and caregivers on performing hemodialysis treatments in a home setting. • Conducted patient assessments, developed individualized care plans, and monitored treatment progress. • Provide clinical support, troubleshoot dialysis equipment, and respond to patient concerns both in-person and remotely. • Collaborate with the interdisciplinary care team to coordinate and optimize patient care. • Ensure accurate documentation and compliance with all regulatory and safety standards. Candidate requirements • Current Registered Nurse (RN) license in the state of Indiana. • Associate or Bachelor of Science in Nursing (ASN or BSN) required. • Minimum of 1 year of nursing experience; previous dialysis or home health experience strongly preferred. • Current BLS certification required; ACLS preferred. • Excellent teaching, assessment, and communication skills. • Valid driver's license and reliable transportation for patient visits and training sessions.
