As a Home Care Registered Nurse, you will deliver and oversee nursing care to patients in their homes, as ordered by physicians and in accordance with all relevant laws, regulations, and agency policies. You will collaborate with other healthcare professionals to ensure cohesive care for each patient by effectively planning and sharing vital information with physicians, agency staff, patients, families, and community resources.

Key Responsibilities:
• Deliver exceptional clinical services following infection control standards and within the scope of practice.
• Coordinate patient care with the healthcare team from admission through discharge.
• Conduct thorough nursing assessments as required by federal and state programs and payer specifications.
• Ensure patient eligibility and medical necessity for services, adhering to payer and agency policies.
• Create and update personalized care plans in collaboration with community providers.
• Review and adjust care plans as necessary, with approval from the physician, to meet the ongoing needs of the patient.

Required Qualifications:
• Valid and unrestricted RN licensure in the state of practice.
• Current driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
• Valid CPR certification.
• Ability to work effectively in diverse home environments without discrimination based on age, race, creed, color, sex, disability, or financial status of the client.

Preferred Qualifications:
• Minimum of 1 year of RN experience.
• Proficient in working independently.
• Strong communication, writing, and organizational skills.

Compensation is based on various factors such as local labor markets, education, work experience, and certifications. In addition to competitive hourly pay ranging from $28.27 to $50.48, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contributions (subject to eligibility requirements).
