Home Care Registered Nurse in Zimmerman, MN

We are seeking a skilled and compassionate Home Care Registered Nurse to join our dedicated team part time 24 - 30 hours per week. This vital role involves delivering skilled nursing care in a home setting to our Veteran and skilled nursing clients, supporting clients’ health and well-being while promoting independence. The ideal candidate will possess a strong clinical background, excellent communication skills, and a passion for enhancing patient quality of life through personalized care. • Complete initial, face to face comprehensive assessment of prospective/new clients and develop a proposed service plan. • Travel to clients’ homes and some travel to meetings and seminars may be required. • Monitor and reassess clients every 60 days after initiation of services. Monitors and reassesses clients as needed based on changes in the clients’ needs. Update client plans of care according to each assessment. • Coordination of client’s home care services with other service providers, such as hospice, and transitions to the hospital or nursing home. • Use and operate Generations Home Care system to assure clients have up-to-date care plans that address their needs and preferences, including interventions to address any areas of vulnerability and individualized medication management or treatment or therapy management plans. • Performed Skilled nursing tasks according to doctors orders. • Be available to provide client care in the event of caregiver absences. • PHYSICAL DEMANDS: • Must be able to move throughout the work day. • Must have adequate vision and hearing to meet the requirements of this position and to perform administrative requirements. • While performing duties of this job, employee is regularly required to use hands and fingers to examine clients, handle or feel objects, tools, or controls. In addition, the job requires walking, bending, stooping, pushing, pulling, kneeling, reaching overhead and lifting up to 30 pounds . • Must be able to cope with the mental and emotional stress of the position. • A graduate of an accredited school of nursing, must be registered and licensed with the State of Minnesota as a RN, and must maintain a current unencumbered license with the Board of Nursing. • At least 1 year of experience in home care or a long term care facility experience is required. • Must receive a “not disqualified” criminal background result from the Department of Family Services and may not be on the OIG exclusions list. • Must be CPR certified. • Must have a valid MN driver’s license. • Ability to use computers and electronic records. • Ability to effectively communicate in English both verbally and in writing. • Knowledge of complex medical conditions including infectious disease care, acute pain management, trauma medicine, and geriatrics. • Excellent communication skills with the ability to educate patients effectively while demonstrating professionalism and empathy. • The ability to deal tactfully and use good communication and customer service skills with clients and their families.
