Work alongside dedicated physicians and healthcare professionals to create and implement effective care plans that meet each patient's unique needs. Your Key Responsibilities: • Conduct patient admissions, manage ongoing cases, and provide skilled nursing follow-up visits in home health environments. • Deliver effective care management as outlined by the Clinical Manager, ensuring important follow-up is completed. • Collaborate with physicians to establish initial treatment plans based on patient assessments and medical orders; provide hands-on care and modify care plans as needed based on ongoing evaluations. • Coordinate care with a multidisciplinary team, including therapists and home health aides, to ensure that patients receive holistic support. • Consistently report patient progress, conditions, and care details to both the patient's physician and the Clinical Manager. • Work together with patients and their families to implement care plans that address health needs effectively. • Supervise Home Health Aides and caregivers to ensure adherence to established care plans, providing necessary documentation and training. • Facilitate the discharge process in partnership with the physician and Clinical Manager, ensuring all clinical documentation is completed accurately. • Prepare thorough medical documentation, including care plan modifications, progress updates, and medication orders to contribute to excellent patient care. What We Are Looking For: • Nursing Diploma, Associate’s Degree, or Bachelor’s Degree. • At least one year of nursing experience preferred. • Experience in medical-surgical, ICU, ER, or acute care settings is advantageous. • Prior home health experience is a plus. • Current, unrestricted Registered Nurse License. • Active CPR certification. • Excellent organizational and communication skills. • Valid driver's license, auto insurance, and reliable transportation. Compensation and Benefits: Compensation ranges from $45.00 to $63.00 per visit/unit, equating to an annual salary of $70,500 - $96,900 for full-time (40 hours per week) work. Adjustments may be made based on location and qualifications.
