Shift Detail: Monday - Friday in various towns including Old Saybrook, Old Lyme, East Lyme, Waterford, New London, Groton, Stonington, and Ledyard.

As a Primary Care Nurse (RN), you will leverage your education and training to deliver personalized care in an environment that fosters independence. This role entails covering the areas of Old Saybrook, Old Lyme, East Lyme, Waterford, New London, Groton, Stonington, and nearby towns.
Key Responsibilities:
• Conduct comprehensive assessments to create individualized care plans.
• Facilitate effective communication among patients and the interdisciplinary team.
• Establish and maintain therapeutic relationships through education and promoting patient self-advocacy and independence.
• Use state-of-the-art technology for documenting the skilled services provided.
Qualifications:
• Valid CT RN license.
• At least one year of RN experience; home care experience is a plus.
• Current driver's license and reliable transportation.
• Proficiency in computer use is essential.
