Position Overview: As a Primary Care Nurse, you will offer intermittent nursing services to patients under a collaboratively developed care plan with their physician. Your role will focus on promoting comfort and quality of life for terminally ill patients while adhering to hospice principles. Key Responsibilities: • Admit and assess patients, developing personalized care plans in partnership with families. • Provide ongoing evaluation and update care plans based on changing patient needs. • Document all clinical services accurately and within regulatory timelines. • Manage a caseload efficiently, ensuring productivity and flexibility to meet unexpected demands. • Supervise and mentor Home Care Aides, ensuring high-quality patient care. • Continuously enhance your professional skills through training and development opportunities. Qualifications: • Current Registered Nurse license in the State of Connecticut. • Graduate of an NLN-approved nursing program (associate or baccalaureate). • One-year experience in medical/surgical nursing; community health nursing is a plus. • Knowledge of hospice/palliative care principles. • Effective communication skills with the ability to provide compassionate patient care.
