Key Responsibilities:
• Conduct comprehensive assessments to create personalized care plans.
• Facilitate clear communication between patients and the interdisciplinary team.
• Build therapeutic relationships by educating patients and encouraging self-advocacy.
• Utilize advanced technology to document skilled nursing services.
Qualifications:
• Registered Nurse with a valid CT RN license.
• Minimum of one year of RN experience; homecare experience preferred.
• Valid driver's license and reliable transportation are required.
• Proficiency in computer skills is necessary.
