Job Description

Overview: We are looking for a compassionate Home Health RN to provide high-quality nursing care to patients in their homes. Key Responsibilities: • Conduct patient assessments and develop individualized care plans. • Administer medications and treatments as prescribed. • Monitor vital signs and document patient progress. • Educate patients and families on care plans and self-care. • Collaborate with healthcare teams for coordinated care. • Provide specialized nursing services (e.g., wound care). • Maintain accurate patient documentation in EHR. • Participate in on-call duties as needed. Qualifications: • Current Maine RN license required. • BSN preferred; associate degree accepted. • Home health care experience preferred. • Strong clinical assessment and communication skills. • BLS certification required; ACLS preferred.
