Status: Full Time; Monday-Friday, 8a-5p, with on-call rotation

As a Home Health Admission RN, your key responsibilities will include:
• Conducting initial and ongoing comprehensive assessments of patients.
• Coordinating services for patients/clients.
• Delivering care per the patient’s/client’s service plan.
• Collaborating with patients/clients to clarify and establish mutually agreed-upon care goals.
• Performing home visits to assess patient conditions, track progress, provide care, and facilitate education and skilled interventions.
• Creating a care plan with the Case Manager that outlines goals based on nursing diagnoses, including varied nursing actions, and involving patients and families in the planning process.
Qualifications for Home Health Registered Nurses include:
• A graduate of an accredited nursing program with an active RN license in Colorado.
• A minimum of one (1) year of nursing experience, preferably in home healthcare.
• OASIS experience preferred.
• Current CPR Certification (demonstration course required).
• Ability to pass a background check and drug test (note: we test for THC).
• Strong capacity to work independently and communicate effectively with patients/families and team members.
• Familiarity with state, federal, and licensure regulations.
• No Covid vaccination requirements.
