Status: Full Time
Salary: $80,000 per year, including potential bonuses
Shifts: Full Time; Monday-Friday, 8a-5p, with on-call rotation

Your Role as a Home Health Admissions RN:
• Perform initial and ongoing comprehensive assessments.
• Coordinate services for patients and clients.
• Deliver care based on the patient's developed plan.
• Engage with patients to set and clarify care goals.
• Conduct home visits, assess conditions and provide necessary care.
• Collaborate with Case Managers to create effective care plans.

Qualifications:
• Graduate from an accredited nursing program with an active RN license in Colorado.
• At least one year of nursing experience, preferably in home healthcare.
• Experience with OASIS is preferred.
• Current CPR Certification.
• Clean background and drug test results (including THC testing).
• Ability to work independently and communicate effectively.
• Knowledge of applicable regulations and licensure requirements.
• No Covid Vaccination Requirements.
