Per-Diem Home Health Care Registered Nurse Location: PLYMOUTH AREA Join our compassionate and dedicated team providing professional care to patients in the comfort of their homes. We are looking for a skilled Home Health Care Registered Nurse who is committed to patient-centered care. What We Offer • Mileage Reimbursement • $5 differential for visits performed on Sunday and Saturday • Supportive team environment Wages • The per visit rate for this position is $27 Responsibilities • Delivers skilled nursing care to patients in their home in accordance with individualized care plans. • Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services. • Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs. • Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards. • Conducts ongoing medical documentation, writing OASIS assessments as appropriate. • Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement. • Responds promptly to emails and phone calls Requirements • Current RN license in the state of Massachusetts • Strong technology skills • Reliable transportation, valid driver's license, auto insurance Apply Now!
