JOB TITLE: Home Health & Hospice Clinical Coordinator/RN DEPARTMENT: Home Health & Hospice Admin REPORTS TO: Home Health Hospice Director JOB SUMMARY The Home Health and Hospice Clinical Coordinator/RN perform records review to assure compliance with Medicare conditions of participation, accurate claims submission and documentation to support services rendered and submitted for reimbursement. This position also oversees staff education and both the Home Health and Hospice Quality Assurance Program (QAPI). The HH & Hospice Clinical Coordinator must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures. Position requires occasional travel throughout the service area; reliable transportation is necessary, with valid driver's license and vehicle insurance. Availability is also required for a rotating call schedule. JOB REQUIREMENTS Currently licensed as a Wyoming Registered Nurse (RN). EXPERIENCE Two years Home Health Nursing. Knowledge of claims processing. Knowledge of the Conditions of Participation for Home Health and Hospice. SKILLS • Must possess a high degree of confidentiality in all matters with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy. • Must have strong interpersonal skills with the ability to communicate with staff and public in a friendly, courteous and helpful manner. • Knowledge of nursing principles. • Must have ability to work autonomously. • Must have the ability to extrapolate critical patient data as it relates to the billing process and for QI data collection. • Able to perform effectively in highly stressful situations. • Must be well motivated, organized and able to prioritize multiple tasks. • Has the ability to assume a leadership role and serve as an example to other employees. • Effective computer and keyboarding skills. • Must have ability to learn and use Microsoft Office Suite products including Word and Outlook. • Ability to learn new computer software applications. REQUIRED CERTIFICATIONS BLS (AHA); Clean valid driver’s license. ESSENTIAL FUNCTIONS • Reviews OASIS data reports through Home Care and submit to the state. • Review HOPE data reports and submit to CMS or other governing bodies as required. • Completes quarterly Performance Improvement activities including data collection, analysis, reporting and providing education related to these activities. • Review Home Health and Hospice charts for claims submission and provide information and data to outsourced billing and coding company. • Assume daily duties of Clinical Supervisor or Director as needed. • Rotating administrative weekend or holiday call to assist clinical staff. • Assist staff with software issues as needed. • Clinical visits as needed for Home Health/Hospice patients. • Share in administrative rounding responsibilities at Spirit Mountain Hospice. • Attend hospital interdepartmental meetings as a resource for Home Health and Hospice questions, referrals and standards. • Comply with all Federal, State and Local regulations. Keep informed and educate staff on new developments and changes with the Medicare Conditions of Participation. • Perform follow-up phone calls on Home Health and Hospice patients. • Participates in departmental administrative duties including, but not limited to, staff supervision, staff scheduling, orientation of new departmental staff and ongoing education and clinical oversight of staff. SECONDARY FUNCTIONS • Participate in in-service programs, discharge planning, case conferences, IDT meetings and staff meetings. • Participate jointly with the Director to interview, hire and orient new staff. • Assist with Time & Attendance duties as needed. • Referral Intake and Coordination of Services in the absence of the Clinical Supervisor. • Assists in promoting community education. • Assist with Quality Improvement activities related to clinical services, including chart reviews. • Participate in the development of programs for the community, including the memorial service, education and fundraising to assist in the community wide awareness of issues related to death and dying. • Assist with department advertising when needed. • Other duties as assigned.
