Home Health Licensed Practical Nurse
Make a meaningful impact every day as a nurse with CenterWell Home Health. Provide personalized, one-on-one care to help patients regain independence in their own homes. Collaborate with a committed team of physicians and clinicians to create and implement care plans that aid in recovery, helping patients return to the activities they enjoy.
Home Health LPN Responsibilities:
• Deliver professional nursing care under the supervision of an RN, following the state's Nurse Practice Act, licensing requirements, healthcare standards, laws, and company policies.
• Execute skilled nursing procedures for patient illness, rehabilitation, and preventive care using a holistic approach tailored to the care plan.
• Apply clinical practice standards to solve problems and achieve specific patient goals.
• Discuss necessary changes in the Treatment Plan with the RN Supervisor and relay physician orders as allowed by state regulations.
• Implement the nursing care plan with an outcome-oriented method.
• Maintain timely communication with patients, doctors, clinical managers, and healthcare team members regarding patient needs and status changes.
• Engage in care coordination and discharge planning as needed.
• Uphold the highest professional conduct regarding confidential information, sharing only when it is clearly appropriate.
• Ensure the availability and proper communication about supplies and equipment necessary for safe and effective care plan execution.
• Complete and maintain documentation as required by the company, ensuring visit/shift notes are recorded the day services are provided.
Use Your Skills to Make a Difference
Essential Experience/Skills:
• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable vehicle.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting, ideally in home health or hospice care.
Compensation
• $28.00 - $40.00 per visit/unit
• $44,600 - $61,400 annual base pay
