Home Health Licensed Practical Nurse (LPN) - Saint Louis, Missouri in St. Louis, MO

Responsibilities: • Conduct skilled nursing visits to assess and implement individualized care plans in home environments • Administer medications, perform wound care, and support clinical treatments including IV therapy • Monitor patient conditions and communicate changes to the interdisciplinary team • Educate patients and caregivers on health management, medications, and self-care • Document patient care accurately using Home Care Home Base (HCHB) software • Ensure adherence to infection control protocols and patient safety standards • Participate in case conferences, quality improvement initiatives, and mandatory training Qualifications: • Active and unrestricted LPN license in Missouri • Minimum of one year experience in home health, hospice, or community-based care • Proficiency with Home Care Home Base (HCHB) electronic charting system • Valid drivers license, reliable insured transportation • Current BLS certification • Strong clinical judgment, effective communication, and the ability to work independently in the field Preferred: • Experience with Medicare-certified home health agencies • Deep understanding of home health compliance and documentation standards • Compassionate, patient-centered care approach Schedule & Contract Type: • Full-time contract position • Monday through Friday, 8:00 AM to 5:00 PM, with occasional schedule flexibility • Approximately 35 patient visits per day (2532 workload points) Benefits: • Weekly pay • Medical, dental, and vision insurance options • 24/7 customer support line • 401(k) retirement options Candidates committed to delivering exceptional home health nursing care and capable of managing an independent caseload are encouraged to apply.
