We are seeking either a part-time or full-time candidate that is passionate about helping people in the community. Being self-directed, dependable, confident in delivering care to a senior population, excellent communication skills, and a loving disposition is important to us and our clients. Carefirst Rehab Home Health was established in 2011 as a clinician owned and operated healthcare system serving 10-counties in the Southern Indiana region. Our goal has always been to place the families we gratefully serve as the top priority in the ever growing complex healthcare process. Responsibilities - • Evaluates client in their home setting and assessed clinical areas of importance • Develops and implements nursing plan of care • Monitor patient’s condition and assess their needs to provide the best possible care • Make routine visits to the patients to evaluate their condition • Coordinates with members of the rehabilitation team, family, and facility optimal service delivery • Medication management, education / training for clients served • Foster a supportive and compassionate environment to maximize results • Participates in remote interdisciplinary meetings as assigned • Submits clinical documentation timely as established by supervisory team • Conducts a physical examination of the patient, including vital signs, physical assessment, mental status, skin inspection, etc Skills - • Understanding of disease process and management of symptoms • Excellent knowledge of nursing care methods and implementation • Ability to multi-task and communicate with an interdisciplinary team • Dependable, confident, and compassionate • Ability to make clinical decisions in an ever changing environment • Excellent written and verbal communication abilities • Open to learning new processes / procedures • Current CPR Certificate • Valid nursing license • **For more information on "WHO WE ARE", please visit our website at www.carefirstrehab.com or check us out on social media via FACEBOOK at Carefirst Rehab Home Health or INSTAGRAM (@carefirst_rehab).
