Job Summary of Home Health Nurse:

This role involves providing direct patient care and support in collaboration with healthcare teams, ensuring residents well - being and safety. The position requires administering treatments and medications, monitoring patient conditions, and maintaining accurate documentation to support high - quality care standards.

Job Duties and Responsibilities of Home Health Nurse:
• Support nursing staff by participating in activities of daily living and standardized patient care duties.
• Execute medication and treatment protocols based on physician directives and care plans.
• Handle pharmacy orders and maintain precise records of all medications and treatments administered.
• Track patient health status and provide updates to licensed nurses, supervisors, and physicians.
• Take note of crucial readings like blood pressure, body temperature, and weight, and monitor the levels of fluid and nutritional intake/output.
• Administer primary upkeep comprising wound sterilization, bandaging, and supervision of intravenous methods if necessary.
• Offer assistance with admissions and ensure the proper documentation is in place during patient registration.
• Inform the appropriate personnel promptly about any incidents, accidents, or modifications in the patient's health condition.

Qualifications and Experience of Home Health Nurse:
• An LPN license that is up to date is a must-have for all applicants.
