Key Responsibilities: • Develop comprehensive problem lists and individualized medical treatment plans for patients. • Provide primary, preventive, urgent, or acute care directly on-site. • Identify and stratify high-risk patients to ensure more frequent clinical visits. • Create contingency plans for managing high-risk health conditions. • Collaborate effectively with residents primary care physicians and facility staff. • Perform routine hands-on bedside care, develop care plans, and manage clinical orders. • Conduct single visits for patients transitioning home as part of their discharge from skilled nursing facilities. Qualifications and Desired Experience: • Master of Science in Nursing (MSN) degree. • Current Oregon Nurse Practitioner license. • Basic Life Support (BLS) certification. • DEA certification. • Prior experience in home health, hospice, or similar settings preferred. • Strong clinical assessment skills and the ability to manage complex patient cases independently.
