
Home Health Nurse Practitioner in Portland, OR in Portland, OR
Job Description
Deliver comprehensive healthcare to senior living and homebound patients in the Portland, OR area by providing expert medical assessments aimed at preventing unnecessary hospitalizations. This contract opportunity offers a Monday through Friday schedule, 8 AM to 5 PM, with no weekends or holidays, allowing for a balanced professional and personal life.
Key Responsibilities:
• Develop individualized problem lists and medical treatment plans for each patient.
• Provide primary, preventive, urgent, and acute care onsite in home and senior living settings.
• Identify high-risk patients and conduct more frequent visits as needed.
• Create contingency plans for managing high-risk conditions effectively.
• Collaborate and communicate with patients' primary care physicians to ensure coordinated care.
• Deliver hands-on bedside care, develop care plans, and coordinate with facility leadership and care teams.
• Conduct post-discharge single visits for patients transitioning home from skilled nursing facilities.
Qualifications and Desired Experience:
• Master of Science in Nursing (MSN) degree.
• Valid Oregon Nurse Practitioner license.
• Basic Life Support (BLS) certification.
• DEA registration to prescribe medications.
• Preferred background in home health, hospice, or related community health settings.
