Home Health Nurse (RN / LPN) – PRN in Gainesville, GA

Job Summary Location: Walton County, GA Job Type: PRN | 1099 We are seeking a skilled Home Health Nurse (RN or LPN) to provide post-hospital skilled nursing visits for a patient recently discharged from Piedmont Atlanta Hospital following a complex medical stay. This is a PRN, intermittent nursing case focused on assessment, monitoring, and education — not daily or continuous care. Job Responsibilities Perform in-home skilled nursing visits following hospital discharge Complete initial nursing assessment (RN) and follow-up visits as ordered Monitor respiratory status, vitals, and signs of infection Medication reconciliation and patient education Monitor nutrition, hydration, and symptom management Reinforce safety, fall prevention, and discharge instructions Communicate changes in condition to provider and care team Complete accurate and timely documentation Important Case Details Recent ICU admission for sepsis and respiratory complications No IV therapy or home infusions Antibiotics, if present, are oral only Nursing is short-term and skilled, with therapy services ongoing Requirements Active Georgia RN or LPN license Home health experience preferred Strong assessment and documentation skills Reliable transportation Ability to work independently Pay RN: $45–$55 per visit LPN: $30–$40 per visit Schedule PRN – pick up visits as needed Flexible scheduling Ideal for supplemental income Job Types: Per diem, Temporary Pay: $45.00 - $55.00 per hour Expected hours: No more than 15 per week Benefits: • Flexible schedule Work Location: In person
