As a Home Health Registered Nurse, your responsibilities will include: • Conducting admissions, case management, and follow-up skilled nursing visits for home health patients. • Providing ongoing care and case management, ensuring necessary follow-up as directed by the Clinical Manager. • Collaborating with physicians to develop initial treatment plans based on physician orders and patient assessments. • Delivering hands-on care, managing and evaluating care plans, and teaching patients in accordance with physician orders under the Clinical Manager's supervision. • Revising care plans in consultation with physicians based on ongoing assessments. • Coordinating appropriate care with healthcare personnel, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers. • Reporting on patient care, conditions, and progress to physicians and Clinical Managers continuously. • Implementing patient care plans with patients and their families to achieve optimal care outcomes. • Supervising Home Health Aides and Personal Care Workers, providing written care instructions that reflect the current care plan. • Discharging patients in consultation with the physician and Clinical Manager, preparing necessary clinical documentation. • Preparing detailed medical documentation for all patients, including case conferences, patient contacts, medication changes, and progress updates. Required Experience/Skills: • Diploma, Associate or Bachelor Degree in Nursing. • At least one year of nursing experience preferred. • Strong medical-surgical, ICU, ER, or acute care experience. • Experience in Home Health is a plus. • Current and unrestricted Registered Nurse licensure. • Current CPR certification. • Excellent organizational and communication skills. • A valid driver's license, auto insurance, and reliable transportation. This position offers: • Scheduled Weekly Hours: 40 • Pay Range: $77,200 - $106,200 per year • Referral Bonus Information: Associates may receive a bonus for referring external candidates, provided all eligibility requirements are met.
