Job Description
As a Home Health Registered Nurse, you will play a vital role in delivering personalized nursing care to patients in their homes, as prescribed by healthcare professionals and in compliance with relevant regulations. You'll work collaboratively with other healthcare team members to ensure comprehensive care and continuity throughout the treatment process.
Key Responsibilities:
• Clinical Excellence
• Develop, implement, and adjust care plans in collaboration with physicians and health professionals.
• Oversee and guide the care provided by home health aides and LPNs, ensuring compliance with state and federal regulations.
• Conduct necessary supervisory visits.
• Documentation and Patient Care
• Deliver high-quality clinical services aligned with patient care plans and infection control standards.
• Perform comprehensive assessments, including OASIS, ensuring accurate medication reconciliation.
• Document patient visits in accordance with agency policy and payer requirements.
• Quality Assurance
• Conduct initial and comprehensive nursing evaluations, confirming eligibility and medical necessity for home health services.
• Maintain open communication with agency staff regarding patient care, visit assignments, and clinical updates.
• Engage with physicians and families to guarantee service excellence and optimal patient care outcomes.
• Team Collaboration
• Work under the guidance of the Clinical Director and Executive Director, completing tasks punctually and professionally.
• Assist in onboarding new team members and mentor staff and students.
• Participate actively in performance improvement initiatives and contribute to patient safety measures.
• Be available for on-call and weekend rotations as necessary to meet patient needs.
• Engage in the agency's utilization management efforts to ensure quality service delivery.

Qualifications:
• Valid and unrestricted RN license.
• Minimum of 1 year of experience in Home Health.
• Current Driver's License, vehicle insurance, and reliable transportation.
• Current CPR Certification.
• Capacity to work in diverse home settings, respecting each client's individual circumstances.
Preferred Skills:
• Ability to work independently and efficiently.
• Strong communication, writing, and organizational proficiencies.
Compensation is competitive and will vary based on factors such as experience, education, and market demand. Our comprehensive benefits package, including 401k and stock purchase options, fosters individual growth. Pay ranges from $28.27 to $50.48 per hour for full-time roles.
