As a Home Health Registered Nurse, you will be responsible for providing and directing nursing care to patients in their homes as prescribed by physicians, while adhering to applicable laws, regulations, and agency policies. This role entails coordinating a comprehensive care plan with other healthcare professionals, ensuring continuity of care through effective communication with physicians, agency personnel, patients, and their families.

Key Responsibilities:
• Clinical Competence:
• Collaboratively initiate, develop, implement, and revise patient care plans with physicians and healthcare professionals.
• Supervise and instruct home health aides and licensed practical/vocational nurses, assigning tasks according to regulations.
• Conduct required supervisory visits.
• Documentation and Care Delivery:
• Deliver high-quality clinical services according to the scope of practice and infection control standards.
• Perform comprehensive assessments (OASIS), including medication reconciliation, accurately and timely.
• Document patient visits in compliance with policy and payer requirements, ensuring timely synchronization.
• Quality Assurance:
• Conduct initial and comprehensive nursing evaluation visits to ensure patient eligibility and develop care plans within state guidelines.
• Effectively communicate relevant information with agency staff regarding patient care issues and coordination efforts.
• Maintain open communication with physicians, patients, and families to guarantee quality care and service excellence.
• Team Collaboration:
• Receive and execute tasks from the Clinical Director and Executive Director in a professional manner.
• Assist with the orientation of new staff members and serve as a mentor to other employees and students.
• Actively participate in performance improvement initiatives aimed at enhancing patient outcomes and safety.
• Engage in on-call and weekend rotations as necessary to meet patient needs.
• Contribute to the agency's utilization management and quality improvement efforts.

Qualifications Required:
• Unrestricted RN licensure in the state of practice.
• At least 1 year of Home Health experience.
• Current driver's license, vehicle insurance, and access to reliable transportation or public transportation.
• Current CPR certification.
• Able to work compassionately and effectively with diverse populations.

Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.

Compensation will be based on various factors such as local labor markets, education, work experience, and certifications. Along with a competitive salary ranging from $28.27 to $50.48 per hour for full-time employment, we offer a comprehensive benefits package, incentive programs, stock purchase options, and 401k contributions, all subject to eligibility requirements.
