
Home Health Registered Nurse in Dallas, PA
Job Description
As a Home Health Registered Nurse, you will play a pivotal role in providing exceptional nursing care directly to patients in their homes, following physician prescriptions and adhering to all relevant regulations and agency policies. Your coordination with other healthcare professionals will ensure seamless patient care and a supportive experience for families and communities alike.
Key Responsibilities:
• Clinical Excellence
• Develop and implement individualized care plans in collaboration with physicians and healthcare teams.
• Supervise and guide the work of home health aides and licensed practical/vocational nurses, ensuring compliance with state and federal regulations.
• Conduct essential supervisory visits to maintain care quality.
• Documentation and Care Delivery
• Deliver high-quality clinical services and ensure infection control standards are met.
• Accurately complete OASIS assessments and medication reconciliations in a timely manner.
• Document patient visits according to policy and payer guidelines.
• Quality Assurance
• Conduct comprehensive nursing assessments to determine eligibility and develop effective care plans in collaboration with physicians.
• Communicate effectively with agency staff about patient care, visit schedules, and adjustments required in ongoing treatment.
• Maintain open lines of communication with physicians, patients, and families to enhance the care experience.
• Team Collaboration
• Follow guidance from Clinical and Executive Directors and complete assigned tasks promptly.
• Help onboard new personnel and serve as a mentor to staff and students.
• Engage in performance improvement initiatives to enhance patient safety and reduce unnecessary hospitalizations.
• Participate in on-call and weekend rotations as necessary to meet patient needs.

Required Qualifications:
• Active and unrestricted RN licensure in your practice state.
• Minimum of 1 year of experience in Home Health.
• Valid Driver's License with vehicle insurance and reliable transportation.
• Current CPR Certification.
• Ability to compassionately serve clients from diverse backgrounds.
Preferred Qualifications:
• Strong ability to work independently.
• Excellent communication, organizational, and writing skills.
We offer a competitive pay range from $28.27 to $50.48 per hour, influenced by local labor markets, education, and experience. In addition to salary, our comprehensive benefits package includes incentives, recognition programs, and a 401k contribution. All benefits are subject to eligibility requirements.
